If both spouses are filing jointly, check the box and enter both spouses' names, emails, and phone numbers.
If married but only one spouse is filing, enter only the filing spouse's contact details. We may still request pay stubs for the non-filing spouse.
Use the filer name and contact information the office should rely on for follow-up.
Leave the case reference blank if the office has not assigned one yet.
Second spouse (required for joint filing)
Only complete this section when both spouses are filing together.
These details help us generate a more accurate missing-document checklist and internal review report.
Each section below has its own drop area. Drag files from your computer, or tap “Take photo” from your phone. Maximum per submission: 60 files and 120 MB total. All uploaded documents are included in our internal AI processing workflow.
You do not need every document before you begin. Upload what you have now and use the same Submission ID later for the rest.
Use the dropdown even if you have not uploaded anything in that category yet. That helps us tell the difference between “not available yet” and “still being gathered.”
Photo ID (driver's license) and SS card
Upload a clear driver's license or other government photo ID and Social Security card. Proof of address is not required.
Pay stubs (last 7 months)
Upload every pay stub from each job for the last 7 months (weekly, biweekly, and monthly stubs). If married and living together but not filing jointly, also include the non-filing spouse's last 7 months of pay stubs.
Tax returns (last 2 years)
Upload full federal and state tax returns for the last 2 years, including schedules and W-2/1099 pages.
Bank and investment account statements (last 3 months)
Upload complete statements (all pages) for checking, savings, brokerage, and retirement/investment accounts for the last 3 months.
Debt notices / collection letters
Upload creditor letters, collection notices, medical bills, and any lawsuit or garnishment paperwork.
Mortgage or vehicle loan statements
Upload your most recent mortgage, auto, or other secured loan statements showing balance and payment.
Any additional documents you'd like to add
Upload any other documents you believe are important for your case (court notices, correspondence, screenshots, or supporting records).
Urgent timing (optional)
Use this only when there is a real deadline or pressure point, such as a garnishment, foreclosure, repossession, hearing, or sale date.
Use this for timing issues, urgent deadlines, missing records, or anything that would help us understand gaps in the file.
