Photo ID (driver's license) and SS card
Upload a clear driver's license or other government photo ID and Social Security card. Proof of address is not required.
Pay stubs (last 6 months)
Upload every pay stub from each job for the last 6 months (weekly, biweekly, semimonthly, and monthly stubs). If married and living together but not filing jointly, also include the non-filing spouse's last 6 months of pay stubs.
Tax returns (latest 2 due years)
Upload full federal and state tax returns for the latest 2 tax years currently due, including schedules and W-2/1099 pages.
Bank and investment account statements (last 3 months)
Upload complete statements (all pages) for checking, savings, brokerage, and retirement/investment accounts for the last 3 months.
Debt notices / collection letters
Upload creditor letters, collection notices, medical bills, and any lawsuit or garnishment paperwork.
Mortgage or vehicle loan statements
Upload your most recent mortgage, auto, or other secured loan statements showing balance and payment.
Any additional documents you'd like to add
Upload any other documents you believe are important for your case (court notices, correspondence, screenshots, or supporting records).
Urgent timing (optional)
Use this only when there is a real deadline or pressure point, such as a garnishment, foreclosure, repossession, hearing, or sale date.
